Who Is Qualified to Act As A SAP?
In order to qualify as and to perform the duties of a Substance Abuse Professional (SAP) under the Department of Transportation (DOT) drug and alcohol testing program, a professional must hold at least one of the following credentials, which must be current and in good standing:

(1) Licensed physician (Doctor of Medicine or Osteopathy);
(2) Llicensed or certified social worker;
(3) Licensed or certified psychologist;
(4) Llicensed or certified employee assistance professional;
(5) State-licensed or certified marriage and family therapist; or
​
(6) Drug and alcohol counselor certified by an organization listed at https://www.transportation.gov/odapc/sap.

Furthermore, they need to successfully complete a DOT-endorsed training program comprising hours of instruction, plus professional development hours (e.g., continuing education units) every 3 years.

How Long Does the DOT Return-To-Duty (RTD) Process Take?
There is no predetermined timeline for completing the Department of Transportation (DOT) Return-to-Duty (RTD) process. Each case is evaluated individually, and the length of the process depends on the Substance Abuse Professional's (SAP) clinical recommendations and your progress in completing the required education and/or treatment.

Under DOT regulations, a SAP cannot establish or promise a specific timeframe for completing the RTD process before conducting a comprehensive initial evaluation. Any SAP who guarantees an expedited process or promises to return you to safety-sensitive duties within a specific timeframe may be misrepresenting the requirements of federal regulations.

Your progress depends on your participation in and compliance with the education and/or treatment recommendations established by your SAP.

What Is the Cost of SAP Services?
SAP fees vary by provider and region. At Willroth Consulting, the fee is $600 per DOT violation. This covers your initial evaluation, referral recommendations to appropriate providers, treatment plan with revisions as needed, treatment monitoring, follow-up evaluation, and final recommendations to your employer. It does not include the cost of treatment or education programs, return-to-duty testing, or follow-up testing. Any future DOT violation requires a new evaluation and fee.

DOT regulations do not specify who pays for SAP services. Your employer or its Employee Assistance Program (EAP) may cover the cost; if not, you are responsible. Please check with your employer for details.
​
Health insurance does not cover SAP evaluations, although some recommended treatment services may be covered under your plan.
​
Payment in full is due before your initial evaluation begins. If payment is not received at your appointment, the evaluation cannot proceed. Several payment options are available and will be reviewed when you schedule. For details, call our office at 501-733-9063.
The SAP process consists of four steps: Initial Evaluation, Treatment/Education recommendation, Follow-Up Evaluation, and the creation of a Follow-Up Testing Plan.
